According to this article the composite blades on this helicopter vibrated when the aircraft was flying at a low weight, as in low fuel and no passengers. This one had 5 PAX but reportedly was low on fuel. I have never heard of this but it may explain why it just fell apart in the air for no apparent reason. It also makes me wonder why the blades were still authorized for use despite knowing the problem.
