When the indictment came down, people were (rightfully) up in arms that no other person having sex with these girls was included as a defendant.
So to answer your question, there does not need to be a third person for the perp to be guilty of sex trafficking. As I said above, its indefensible that the DoJ is not going after anyone else. But that does not mean anyone is claiming Epstein was innocent.

Isn't it a question of jurisdiction? Was Epstein island under the jurisdiction of the US government? Can you charge someone for a crime they commit in another country where something is legal?
Jurisdiction is not an issue here. The acts in the indictment all occurred in the USA (NY and FL). When it came down, people were also up in arms (again rightfully) that nothing was mentioned about the Island.

But that would negate the whole point of compiling it in the first place (holding the threat of releasing it over their heads so they do what you want/need)
Such a list is not being withheld for the protection of those who might be on it, its for the protection of our intelligence services who either perpetrated it, allowed it, or were clueless about it.
